Pasteurization of Milk From an HIV-Infected Woman

چکیده

Drs Giles and Mijch have provided a timely case of an HIVinfected woman elected to exclusively breastfeed applying milk pasteurization techniques [1]. This is the first report of such a case in a developed country. We would appreciate the opportunity to discuss the effect of pasteurization in this paper. HIV RNA was detected at <250 copies/mL in the first paired specimens of breast milk, both preand postpasteurization. Four months later, the breast milk sample prepasteurization had 60 copies/mL and the sample postpasteurization had 80 copies/mL. If the authors have a proof of a well-done pasteurization, these results challenge our knowledge about HIV inactivation by heat treatment of human milk because this method, including devices that can be used in a home setting, has shown to decrease the infectious titer of cell-free HIV-1 by more than 5 logs [2]. An important problem with regard to HIV-infected patients is compliance with treatment. In fact, in this case, the patient ceased her own antiretroviral medication before this pregnancy. By the way, another explanation for these results is that this patient had neglected the pasteurization of her breast milk, because postpasteurization specimens reported the same or an increase in copy number with respect to prepasteurization specimens. If this is the case, Giles and Mijch have taught us a lot about how to address the remaining challenge of prevention of mother-to-child transmission of HIV through breastfeeding in developed countries. The way forward to treat these cases must include careful selection of patients to make sure of the compliance with treatment, milk bank pasteurization to strengthen the prohibition of using raw mother’s milk, and monthly blood test of the baby to discard any increase in copy number. There is place for physicians to support HIV-infected women willing to breastfeed in rich countries, under strict control.

Pasteurization of Breastmilk

New research from California confirms the safety of pasteurized mother’s milk for HIV-exposed babies (Israel-Ballard, 2005). Two methods of heat-treatment, known as Flash-Heating and Pretoria Pasteurization, were compared to assess their impact on HIV and antimicrobial properties of breast milk, as well as on nutrient content.Milk samples were spiked with clade C HIV-1 (the sub-type found in ar...

Pasteurization of human milk to prevent transmission of Chagas disease.

Although admittedly transmission of Trypanosoma cruzi infection through breastfeeding is a rare event, it involves serious risks. To test the effectiveness of pasteurization in preventing this mode of infection, three sets of samples of human milk were tested: a - contaminated with T. cruzi and pasteurized; b - contaminated with T. cruzi and non-pasteurized; c - non-contaminated and pasteurized...

On-farm batch pasteurization destroys Mycobacterium paratuberculosis in waste milk.

A recent dairy survey conducted in 1996 by the National Animal Health Monitoring System suggests between 20 and 40% of dairy herds in the United States have some level of Johne's disease. This figure will continue to increase unless producers implement management regimes that will help control the spread of this disease within their herds. The neonatal calf is the target for infection with Myco...

Efficacy of pasteurization conditions for the inactivation of Mycobacterium avium subsp. paratuberculosis in milk.

Mycobacterium avium subsp. paratuberculosis, the causative agent of a chronic enteritis in ruminants (Johne's disease), has been linked to Crohn's disease in humans. This microorganism is shed by infected animals primarily in the feces but is also shed in the milk at much lower levels. Therefore, dairy products from infected animals may be one mode of transmission of this animal pathogen. This ...

Stability of a Tick-Borne Flavivirus in Milk

The tick-borne flaviviruses (TBFV) occur worldwide and the tick-borne encephalitis virus (TBEV) members of the group often cause severe, debilitating neurological disease in humans. Although the primary route of infection is through the bite of an infected tick, alimentary infection through the consumption of TBEV-contaminated dairy products is also well-documented and is responsible for some d...
